Justice-Involved Family Navigator (50%)
Status: Part-Time, 20 hours per week; Non-Exempt
Location: The Matthews House, Fort Collins, CO; hybrid option up to 20% of total work hours
Pay Range: $22.00-23.00 per hour hour
JOB DESCRIPTION:
The Justice-Involved Family Navigator is a hands-on, full-time role supporting individuals and families impacted by the criminal justice system through individualized, strengths-based, and trauma-informed case management. This position helps participants navigate court, probation, behavioral health, housing, employment, education, and family systems while reducing barriers to stability and recidivism. By building trusting relationships and coordinating care across justice, social service, and community partners, the Navigator supports participants as they move toward accountability, healing, and long-term success.
QUALIFICATIONS:
• An associate degree in Criminal Justice or Social Work is required; a bachelor’s degree in Psychology, Social Work, Child Development, Criminal Justice, Sociology, or Counseling is preferred.
• 1+ years of experience in case management, social services, behavioral health, or a related human services role is required.
• 2+ years of experience working with justice-involved populations, probation/parole systems, or court-involved family or youth programs is preferred.
- Preferred to hire someone with lived experience in the justice system and/or substance use and recovery
• First Aid/CPR certification is required.
• Strong ability to apply trauma-informed, person-centered approaches that promote accountability, dignity, and positive behavior change.
• Strong skills in navigating court processes, legal systems, housing, safety, and compliance requirements.
• Ability to maintain accurate documentation and manage caseloads, court-related deadlines, and reporting requirements in a fast-paced environment.
• Strong communication, advocacy, collaboration, and professional-boundary skills in complex, high-stakes situations.
• Reliable transportation is required; up to 40% travel may be expected.
ESSENTIAL DUTIES AND MAJOR RESPONSIBILITIES
Individual Case Management & Goal Planning – 30%
• Conduct intakes, assessments, and individualized service planning with justice-involved participants.
• Facilitate regular one-on-one check-ins focused on housing stability, employment, education, behavioral health, family reunification, and compliance with court or probation requirements.
Care Coordination & Community Partnerships – 25%
• Connect participants to housing, workforce, behavioral health, substance use treatment, healthcare, and family support services.
• Collaborate with internal TMH programs and external partners to ensure continuity of care.
Documentation, Data & Compliance – 20%
• Maintain accurate case notes and required reporting in Community Care Link (CCL), Salesforce, and justice-system reporting tools.
• Track participant outcomes and compliance with grant and contract requirements.
Systems Navigation & Legal Coordination – 15%
• Support participants during court appointments by providing guidance and assistance as they navigate the legal system.
• Help families access resources for legal actions or motions that promote healthier futures.
Program & Team Support – 10%
• Participate in case staffing, trainings, and collaborative meetings.
• Support outreach and engagement with referral partners, including community-based organizations. Assist with other duties as assigned.
SCHEDULE:
• Part-time, 20 hours per week, generally Monday through Friday; hours may vary based on participant and program needs.
• Some evenings and weekends may be required to meet the needs of participants and the position.
• This role is based at The Matthews House in Fort Collins and includes community-based work, court-related appointments, and travel to partner locations.
